confirmation of your booking from us
All passengers must complete the following forms.
1.) The Expedition
Application must be signed and sent by post or fax. If it is within 60 days of your
charter, please use email or fax.

2.) The second part is the Liability Release and Diver or Non-Diver Liability Agreement.
This part must be signed and sent by post or fax. If it is within 60 days of your charter,
please use fax.

Pay the balance owing for your expedition, 60 days prior to travel.
To fill out the form electronically, you

Payment and cancellation policy :
A 50% deposit is required to make a booking with the balance payable 60 days prior to
departure. Full payment is required for bookings less than 60 days prior to departures
strongly recommend you take out travel insurance, trip cancellation and Emergency
Medical Evacuation Assistance, (see DAN Information sheet), prior to your departure.
